About the Role:
As a Personal Adviser, you will play a vital role in helping young people achieve positive outcomes in areas such as education, employment, housing, health, and emotional wellbeing.
Key responsibilities include:
* Managing a caseload of care leavers as their key worker.
* Co‑working with Social Workers to contribute to effective care planning for Children in care preparing to leave care.
* Developing, implementing, and reviewing Pathway Plans within statutory timescales.
* Delivering outcome‑focused direct work, individually and in groups, to build young people’s skills and independence.
* Ensuring robust contingency planning to support young people’s stability.
* Encouraging young people to maintain family and community connections, access health, education, training, and employment opportunities.
* Promoting young people’s active involvement in service development and decision‑making.
* Collaborating with multi‑agency partners to secure resources and support.
* Preparing reports and attending statutory meetings, including reviews and court hearings.
* Advocating on behalf of young people and ensuring they are aware of their rights and complaints procedures.
We’re looking for someone who is:
* Committed to improving outcomes for care‑experienced young people.
* Skilled in engaging and empowering young people from diverse backgrounds.
* Knowledgeable about the Children (Leaving Care) Act 2000 and related legislation.
* Organised, resilient, and able to manage a caseload effectively.
* A team player with excellent communication and partnership working skills.
