3 Month Contract With A Local Authority
Role Purpose
The Family Support Worker will work directly with children and young people with a wide range of disabilities and their families, providing coordinated support tailored to assessed needs. The role focuses on ensuring that support packages remain appropriate, effective, and responsive through regular review, multi-agency collaboration, and proactive case management.
The post holder will drive progress against agreed plans, act as a key point of contact for families and professionals, and ensure children and young people receive timely, coordinated, and outcome-focused support.
Key Responsibilities
* Hold a caseload of children and young people with a variety of disabilities who are accessing assessed support packages.
* Build positive, trusting relationships with children, young people, and their families, ensuring their voices are heard and central to planning.
* Regularly review support packages to ensure they continue to meet the evolving needs of the child and family.
* Identify emerging needs or risks and take appropriate action in line with policies and procedures.
* Drive progress for each child and young person within their individual plan, ensuring outcomes are clear, measurable, and reviewed regularly.
* Coordinate, chair, and record multi-agency meetings such as Team Around the Child (TAC) meetings.
* Participate in and contribute to relevant forums, including Education, Health, and other statutory or professional meetings, as required.
* Ensure plans are updated in a timely manner and reflect professional input and family views.
* Liaise effectively with a wide professional network, including education, health, social care, and voluntary sector partners.
* Act as a central point of coordination to ensure joined-up support and clear communication between professionals and families.
* Share relevant information appropriately and in line with data protection and safeguarding requirements.
* Promote the welfare and safeguarding of children and young people at all times, following organisational and statutory safeguarding procedures.
* Maintain accurate, timely, and high-quality case records in line with organisational standards.
* Work within professional boundaries and uphold confidentiality.
* Attend the office in Chessington in line with attendance requirements.
* Participate fully in induction, training, supervision, and team meetings.
* Contribute to service development and continuous improvement initiatives.
Requirements
* Experience working with children and young people with disabilities and their families.
* Strong understanding of child-centred planning and family-focused practice.
* Experience of multi-agency working and coordinating professional input.
* Ability to chair meetings and manage complex discussions effectively.
* Excellent communication, organisational, and interpersonal skills.
* Ability to manage a varied caseload and prioritise work effectively.
* Knowledge of safeguarding and child protection procedures.
* Relevant qualification in health, social care, education, or a related field.
* Experience of chairing Team Around the Child or similar multi-agency meetings.
* Knowledge of local authority or SEND processes and frameworks.
