Overview
To provide a clinical pharmacy service to the Respiratory Speciality within the Medicine directorate at RCHT. Providing core clinical pharmacy service to respiratory inpatients in line with the Trust and clinical pharmacy standards.
Ensuring high quality prescribing and care is provided on and during admission through to discharge to respiratory patients by effective clinical screening. Ensuring that pharmaceutical knowledge and skills are applied through medication reconciliation on admission, optimisation during their admission and input to planning for a successful discharge process where appropriate.
Identify medicine-related causes for admission and develop treatment plans to optimise care.
