Role Overview
As a Community Care Coordinator Apprentice, you will learn how to support the coordination and supervision of homecare services. This role combines office-based training with practical field experience, giving you the skills and knowledge to progress into a permanent coordinator role upon completion.
Key Responsibilities (training provided)
· Assist with client assessments and care reviews.
· Help develop and update care plans and risk assessments using digital care systems.
· Learn how to monitor care delivery and support changes in client needs.
· Observe and practice communication with clients, families, and healthcare professionals.
· Support rota planning and scheduling, matching carers to client preferences.
· Shadow supervisors in mentoring, staff checks, and inductions.
· Gain knowledge of safeguarding procedures and compliance requirements.
· Participate in the on-call rota under supervision as your training progresses.
Person Specification
· Enthusiastic about starting a career in health and social care.
· Interest in developing skills in care planning, coordination, and staff supervision.
· Good IT and communication skills.
· Organised, proactive, and keen to learn in a fast-paced environment.
· Committed to completing a funded Level 4 Health & Social Care qualification.
· Flexible and willing to support both office and frontline care activities.
Training & Development
· Fully funded Level 4 Health & Social Care qualification.
· Structured on-the-job training with experienced coordinators.
· Mentoring, supervision, and regular performance reviews.
· Clear progression pathway into permanent Care Coordinator roles.
