Coordinate and deliver appropriate care services aligned with departmental priorities, budget constraints, and best value principles. Collaborate with partner agencies to safeguard vulnerable individuals in line with statutory duties. Manage a caseload effectively, supporting individuals to achieve their goals and build on their strengths. Regularly monitor and review care plans to ensure services remain effective and responsive to changing needs. Maintain accurate and accessible case records using both paper and digital systems, in compliance with departmental policies.
