An exciting opportunity has arisen for an enthusiastic and dynamic individual to join our team as a Lung Cancer Patient Navigator. The role will support the Respiratory service to facilitate the smooth running of the patient journey, ensuring that the lung cancer pathway is coordinated, streamlined, and all processes revolve around the patient and their individual needs.
Role Details
* 12‑month fixed post due to funding.
Responsibilities
* Support the Respiratory service and facilitate the smooth running of the patient journey.
* Ensure the lung cancer pathway is coordinated, streamlined, and patient‑centred.
* Capture and record information about patients in the patient record and Trust systems, keeping it up‑to‑date and accurate.
* Provide specialist knowledge for the pathway of lung cancer patients.
* Utilise excellent communication and organisational skills, along with a high attention to detail, to achieve accurate documentation.
