Overview
We're looking for a TB Specialist Nurse to join our team which treats and supports individuals with TB in Hackney and the City. The TB team has won national awards for its outreach work. We are keen to appoint a candidate who would be strongly interested in working with our case worker to continue the implementation of outreach screening initiatives with hard to reach populations.
Responsibilities
* In collaboration with the team: be responsible for the assessment of care needs and the development, implementation, and evaluation of programs of care.
* Develop a specialist interest within the TB service to provide a high-quality service for residents of Hackney and the City who have TB.
* Conduct home visits of patients and hospital visitation.
* Driving licence is essential due to home visiting, community screening and outreach work.
* Participate in screening programs based at the hospital and in the community.
* Be involved in audit, research and health education across acute and community settings.
* Assist the band 8 and 7 nurses in managing their own caseloads initially; expected to manage own caseload after the training period.
* This band 6 post is a training post aimed at developing knowledge and skills of nurses with some respiratory experience towards a Clinical Nurse Specialist role.

Clinical Practice and Professional Development
* Demonstrates high levels of competence as a clinical nurse, utilising clinical and social assessment skills.
* Participate in Nurse Led Clinics and screening exercises conducted in the community.
* Assess patients’ needs and plan care in accordance with clinic protocols and national guidelines (in consultation with G/H grade nurses).
* Educate and support patients throughout treatment to maximise adherence, providing particular support to those on Directly Observed Therapy (DOT).
* Develop a specialist interest in an area of TB control (e.g., DOT in community and hospital settings including contact tracing).
* Select, carry out and interpret results of TB and HIV tests (e.g., Mantoux test, blood tests/phlebotomy).
* Provide support to patients through a regular home visiting program.
* Learn about TB medications and side effects to assist in managing patient therapy.
* Assist the case manager with specific clients by providing care as directed and in collaboration with senior nursing colleagues.
