Opportunity for 3 sessions Shared Care Salaried GP

We are looking for a Salaried GP who will work closely with the primary care mental health practitioners, including independent prescribing nurses and clinical pharmacists, with a specialist interest in neurodevelopmental assessment and treatment.


Main duties of the job

The purpose of this role is to provide general practice input into the process of initial screening, referral for neurodevelopmental assessment, and ongoing support in the form of shared care with secondary care providers. The GP will have input into the physical health monitoring of people with mental illness and neurodevelopmental disorders, including those with eating disorders, where the patient remains under the care of specialist community teams. They will also provide medical input, support, and supervision for other members of the primary care mental health service where needed.

Job responsibilities

Duties and Responsibilities

The day-to-day treatment decisions, including prescribing, will be carried out by specialist practitioners in the primary care team, with the GP post holder advising predominantly on comorbid physical health conditions, acting on abnormal physical health monitoring results, and participating in multidisciplinary care for complex cases. The post holder will escalate and access further care when needed from specialist teams in the acute trust and community services.

The GP post holder and primary care specialists will have rapid and priority access to secondary care specialist mental health and neurodiversity services for help, advice, and escalation of care when required.

The GP post holder will attend regular and ad hoc (in-person or remote) meetings with other clinicians, either alone or with patients and carers.

The GP post holder will develop and maintain a close relationship with local secondary care services, including occupational therapy, educational psychology, and specialist speech and language therapy.

They should be confident and competent in performing physical health assessments, have knowledge of shared care agreements, and understand medications used for ADHD and mental health conditions, as well as the physical health aspects of common mental health issues.

The postholder is not expected to be a specialist in mental health, eating disorders, ADHD, or neurodevelopmental disorders but will apply their generalist skills to the care of this patient group. Risks will be managed appropriately through escalation to secondary care.

The GP post holder will act as the signatory to shared care agreements between the specialist trust and the primary care provider (STHC), representing the organisation clinically.

Referrals: Patients are accepted via requests from primary care, step-down from specialist services, and referrals from other partner agencies where necessary.

Governance: The GP post holder works autonomously as a GP, employed by STHC, and in partnership with other clinical professionals employed by NHS organizations, including mental health trusts and acute and community trusts. They will liaise and coordinate care with GP practices within South Tyneside as needed.

The post holder will undertake ongoing professional development and engage in activities related to mental healthcare and ADHD management.
