We are seeking a motivated Critical Care Rehabilitation Clinical Nurse Specialist to coordinate, lead, and further develop the Critical Care Rehabilitation Service within the Critical Care Department at The Dudley Group. This senior clinical role will provide expert clinical leadership and oversight to ensure the comprehensive rehabilitation needs of critically ill adults are met throughout their critical care admission and recovery pathway.
The post holder will hold a postgraduate Critical Care qualification, have extensive experience caring for and managing critically ill patients, and demonstrate knowledge of the critical care rehabilitation journey.
Responsibilities
* Work as part of the multidisciplinary team, liaising with internal and external disciplines and agencies to plan and deliver patient-centred care.
* Provide holistic rehabilitation care, recognising and responding to the individual needs of patients and their relatives.
* Assess, plan, implement and evaluate rehabilitation programmes in line with the NMC Code, Trust policies, procedures and standards.
* Monitor critically ill patients throughout their entire journey to ensure access to enhanced rehabilitation.
* Maintain accurate, timely and comprehensive clinical records as an autonomous practitioner.
* Ensure the safe, secure and appropriate use of all rehabilitation equipment.
* Act as a point of contact for staff regarding rehabilitation pathways and patient care planning.
* Ensure patients are actively involved in decisions about their care and are provided with appropriate information.
Qualifications
Postgraduate Critical Care qualification, extensive experience caring for and managing critically ill patients, and knowledge of the critical care rehabilitation journey.
