This is an appointment for a Clinical Pharmacist on a fixed-term 12-month contract to work within our well-established PCN pharmacy team across the Network Area. The role will be based between the neighbourhoods, responding to the needs of the member practices. The post holder will be a pharmacist who acts within their professional boundaries, providing clinical support within the PCN and working as part of multidisciplinary teams in general practice. In this role, they will receive appropriate clinical supervision.
You will ensure that the practices integrate with community and hospital pharmacy to help utilise the skills mix within teams, improve patient outcomes, ensure better access to healthcare, and help manage workload. Manage elements of the quality and outcomes framework and enhanced services such as Quality Improvement. Perform a patient-facing role to include undertaking clinical medication reviews to proactively manage patients with complex polypharmacy in various settings including the surgery, patient homes, and residential care, with a view to meeting the demands of new workstreams in primary care. Represent the PCN to strategic partners, as part of a team, on medicines optimisation and quality improvement. Provide primary support to general practice staff regarding prescription and medication queries, offering expertise in clinical medicines advice. Be supported to develop their role to become a non-medical prescriber if required.

Person Specification
Qualifications
* Undergraduate degree in pharmacy and be registered with the General Pharmaceutical Council.
Experience
* Experience and awareness of the breadth of common acute and long-term conditions likely to be seen in a general medical practice.
