Domiciliary Care and Reablement Team Leader

About the role:
To work under the direction of the Deputy Manager Registered services for Reablement, Domiciliary Care Services, on a locality level. To ensure services are compliant with Regulation and Inspection of Social Care Act 2016 (RISCA 2016). To be responsible for supporting the frontline workers to deliver person centred support in accordance with the RISCA regulations 2016. To line manage, Reablement Care and support workers to promote wellbeing, early intervention, and prevention to deliver outcomes focused care and support through co produced strength- based personal plans.
About you:
• Good communication skills both written and orally including the ability to communicate with individuals, carers, partners organisations and other professionals.
• To disseminate information and mentor the team.
• To work both independently and within a team
• Ability to ensure good reporting and recording of information.
• Good due diligence to be able to read and process data.
• Use initiative, make decisions, problem solve and seek advice /guidance when needed.
• Good Organisational and administrative skills, including rostering, chairing meetings.
• Ability to communicate with individuals, carers, and families in an appropriate manner.
• ICT literate (emails, teams, databases, organisational systems)
• Ability to communicate in an effective and supportive manner This will include an ability to speak, listen, write and use ICT effectively.
• Ability to lead and support a staff team.
• Ability to work using own initiative.
• To establish good working relationships with staff and service user based on respect and openness.
What you will do:
• To coproduce and review personal plans with service users, family and carers to promote independence, positive risk taking and deliver outcomes in accordance with legislation. To support the team in delivering person centred outcomes, positive risk taking and quality of service.
• To supervise and line manage a team of reablement and care support workers to ensure effective service delivery which includes supervision, coaching, mentoring, guidance and promoting progression.
• To ensure you and your staff team are compliant with Social Care Wales
registration requirements and enable individual development in accordance with Adult’s Services policies and legislation, such as Social Care Wales induction framework/PCC induction and relevant QCF qualifications.
• To support team members when working with individuals including those who have complex needs, and behaviours which may challenge. To use own initiative but seek advice and guidance from other professionals available when needed or escalate to managers for further guidance.
• To communicate effectively, confidently, and collaboratively with the team, professionals both within and outside the organisation including other statutory and third sector bodies to facilitate best outcomes for people.
• To ensure the team have working environments that enable them to be
responsive to the service users and effective delivery of the service. To
complete, review required risk assessments and complete and monitor safe systems of work.
