Pertemps Social Care is supporting Redbridge London Borough Council by recruiting for an experienced social worker. Working in a fast-paced team environment, you will be committed to improving the care of vulnerable adults. You will need to work well in a team setting and have excellent interpersonal and written skills.
The role of a social worker involves supporting the discharge of residents from local acute hospitals, out-of-borough hospitals, and the local Rehabilitation Unit. The team operates in line with the Discharge to Assess model of care to coordinate services for discharge pathways. The ideal candidate should have previous experience in adult social services, with a keen interest in hospital discharges. Do you possess strong multidisciplinary working skills?
Location: Ilford
Rate: £29 per hour
Hours: 36 per week
Key Responsibilities:
1. Assessments
2. Undertaking Mental Capacity Assessments
3. Knowledge of relevant social care legislation
4. Safeguarding and the continuing healthcare process
Candidate Requirements:
* A degree in Social Work
* Registration with Social Work England (SWE)
* At least 3 years post-qualifying experience
