Your role will involve:
* Provide a professional Occupational Therapy / Reablement Service, including statutory Social Care responsibilities.
* Contribute to Service delivery that is person-centred and outcome focused with an emphasis on encouraging independence and enabling recovery.
* Work collaboratively with other professionals and agencies to address complex needs and achieve best outcomes for individuals and their carers.
* Work with health colleagues to ensure safe and timely discharges from hospital and step‑down beds in the community.
Person Specification
Experience
* Experience of working with vulnerable adults.
* Good communication and interpersonal skills, with the ability to engage with client, families, and other professionals.
* Ability to work autonomously and manage competing priorities.
* Experienced in coordinating/liaising with multiple agencies/organisations.
* Implement evidence-based practice, keeping abreast of emerging research, best practices, and national frameworks (e.g., NICE guidelines).
Qualifications
* BSc (Hons) or MSc in Occupational Therapy.
* Current registration with the Health and Care Professions Council (HCPC).
* Membership with the Royal College of Occupational Therapists (RCOT).
