Job overview

We are looking for a Consultant ACP to join our Trust supporting site across East and West Kent.

The Consultant Practitioner for Community Hospitals co-leads the medical provision for Community Hospitals, alongside the Consultant Clinical Lead.

This includes medical management of community hospital services across east and west Kent; development of the current and future medical model; and participation in service redesign.

It would be particularly suitable for a forward thinking, innovative individual seeking an exciting opportunity to help plan and deliver modern dynamic Inpatient Frailty services.

Main duties of the job

You will be a clinical expert in Frailty and will fulfil the 4 domains of Advanced Clinical Practice:

1. Advanced and expert level clinical practice within the field of frailty ensuring clinical excellence
2. Leadership and consultancy providing expert professional advice to patients, carers and colleagues
3. Development and provision of education and training to other staff and trainees
4. Service development, evaluation, research and audit.

Qualifications

Essential criteria

5. First level Registration with NMC, HPCP or GPHC
6. Significant post registration experience, and experience as an ACP at band 8a or above
7. Educated to Master’s level or higher degree
8. Research qualification
9. Teaching qualification
10. Leadership
11. Non- medical Prescribing

Desirable criteria

12. Psychology or counselling experience and training/ education

Essential criteria

13. Extensive clinical experience in the care and treatment of people with frailty (including long term conditions and dementia care experience)
14. Advanced skills in history taking and physical assessment of frail people
15. Independent prescribing experience
16. Experience of strategy development and implementation
17. Experience of advanced specialist interventions in the assessment and management of older patients
18. Experience with evidence of audit, research and innovation applied in practice
19. Experience of teaching and education provision
20. Extensive experience in report writing
21. Experience of planning, implementation and delivery of educational programmes relating to long term conditions
22. Advanced IT skills
23. Experience of working across organisational boundaries with partners (commissioning, social care and education providers)
