The chance for an experienced Care Co-ordinator/Care Team Leader to support a dedicated team throughout the days at our Somerset care home, Oaktree Court
As a Care Coordinator you will work closely with the nursing team and the Clinical Manager to provide safe and effective care, working in collaboration with members of the care team by effective communication and leading by example. To recognise and work within the limits of your competence in accordance with current professional practice and support the team to ensure that the care being delivered meets the needs of the people you look after. To demonstrate a clear understanding of person-centred care and that the needs, priorities, and preferences of people are always valued. To demonstrate effective approaches to monitoring signs and symptoms of physical, behavioural, and emotional distress. To have an in depth understanding evident in theory and practice of safe administration of medication reflective of the NMC and CQC guidelines on the Safe Administration of Medications in Health and Social Care. To use electronic systems for Care Plans, Medication, Rotas and training. Full training will be given for all systems. The Care Coordinator has a crucial role to play in helping to keep and raise standards of care and quality of life of all residents within the home.

About you:
Your role as a Care Coordinator will be challenging, varied and rewarding. You will be a passionate, supportive, and a motivated individual with team leading skills. You will lead from the heart. You will get to know the residents, empower, and support them, building good relations with them, their families, and the teams you work with. You will have good knowledge of Medication Management and will follow Majesticare's policies and procedures. You will have a good understanding of communication and confidentiality and not share any private information with others. You hold minimum a Level 2 in Health and Social Care, and you will be willing to work towards a Level 3. You will be fully supported with additional training and personal development necessary for the completion of your duties. As a key member of the Majesticare family you will attend team meetings and other meetings as appropriate, you may be required to undertake additional duties as reasonably requested by the person in charge.
* An understanding of the CQC fundamental standards and quality statements.
* Experience in medication handling and administration
* Understanding of Care Planning
* Relevant vocational Qualification (Level 2 achieved and willing to work towards level 3)
