These Community Link Worker posts are community based positions within the Mid & Upper Nithsdale Home Team which works to improve the health and wellbeing of the local population. This role prioritises the improvement of the health of the local population and is responsive to the needs, interests and circumstances of local people in the local community, helping individuals and communities to have more control over decisions that affect them and promoting health equity, equality and justice.
Within the role, you will be working alongside colleagues to:
· Take a person-centred approach to support people to identify their own personal priorities and goals, to overcome any barriers to addressing these and to link them with local services and activities.
· Support delivery of community health and wellbeing activity
· Contribute to the delivery of the Community Health and Social Care Priorities, and Public Health Priorities for Scotland
· Contribute to reducing health inequalities

This post will involve clinic and home visits in remote and rural areas. A current driving licence is an essential requirement of this post. However, if you have a disability which precludes you from holding a driving licence, consideration will be given if alternative transport can be arranged. Please detail how you would facilitate travel to clinics and home visits in your application.
