What you will do:
* Assess the needs of children with disabilities and their families to ensure positive outcomes.
* Develop and implement tailored, person-centred care plans.
* Work collaboratively with multidisciplinary teams to deliver integrated support.
* Advocate for children and families, ensuring their voices are heard.
* Review and update care plans regularly to maintain effectiveness.
* Maintain accurate and compliant records in line with statutory requirements.
What we’re looking for:
* Qualified Social Worker with relevant experience in children’s services, ideally within disabilities.
* Strong assessment, care planning, and safeguarding skills.
* Ability to work effectively both independently and within a team.
* Excellent communication and organisational abilities.
