Would you be interested in a career within the NHS as a Health Care Assistant in the community? Are you passionate about providing a patient centred support service and want to deliver excellent and compassionate care to people over the age of 18 that live in Bedfordshire?
We are looking for a dynamic, motivated and enthusiastic person who has excellent communication, IT literate and problem solving skills alongside a passion for patient centred care.
Working in the MDT
You will be part of a MDT team in which you will receive clinical and managerial support, which is also part of a wider MDT team that is aligned to social care professionals in reflection of the NHS 10 year plan.
Responsibilities
* You will be responsible for delivering quality care to a designated caseload of people in the place that they call home, you will be following a care plan devised by the community nursing staff who you will be working alongside.
* You will be asked to feedback concise information in a daily handover to ensure changing needs are met and risks escalated.
* The successful candidate will be expected to be flexible and assist colleagues to cover service needs, which will include working across the wider integrated team.
* Access to a car in order to carry out service duties is essential.
