Connect Community Church is pleased to recruit a Housing First Support Worker (35 hours per week). You will be part of a dynamic team supporting vulnerable adults across Somerset, primarily based in Wells. The individuals have complex needs and are experiencing homelessness.
POST: Housing First Support Worker
CONTRACT TYPE: Full-time, 35 hours/week (subject to annual funding reviews). Job share considered.
SALARY: £26,351.52 per annum
DEPARTMENT: Connect Centre – Housing First
LOCATION: Based in Wells, serving the Somerset area, mainly Mendip District.
BACKGROUND: Connect Community Church is part of the Foursquare Gospel Alliance, a Christian denomination committed to serving the community's spiritual, social, physical, mental, and emotional needs in an accessible way.
KEY OVERVIEW:
Reporting to the Housing First Team Leader, the Housing First Support Worker provides personalized, trauma-informed support to individuals with complex needs facing multiple exclusions, aligned with Housing First principles. The role involves creating long-term, sustainable tenancies by empowering clients, fostering independence, and enhancing community connections.
ADDITIONAL INFORMATION:
* Valid driver’s license and access to a car for work purposes.
* Working pattern: Monday to Friday, with occasional weekend work.
* Includes a 6-month probation period.
KEY RESPONSIBILITIES:
1. Deliver intensive, trauma-informed support within clients’ own tenancies.
2. Assist clients in articulating goals, accessing community resources, and developing peer networks and leisure activities.
3. Maintain accurate records on the Inform case management system.
4. Review and update support plans regularly.
5. Build effective relationships with external partners and promote Housing First initiatives.
6. Liaise with landlords to support tenancy stability, including practical help with furniture, benefits advice, and accompanied visits.
7. Coordinate with agencies on issues like substance misuse, mental health, social care, and employment, with client consent.
8. Encourage peer involvement within the service.
9. Conduct dynamic risk assessments and follow lone working safety procedures.
10. Follow safeguarding procedures with a positive risk-taking approach.
11. Identify and address barriers to housing and tenancy management.
12. Contribute to service evaluation reports.
13. Engage in reflective practice for wellbeing and development.
14. Record petty cash and project expenditures as per policy.
15. Work within health and safety policies, including lone working procedures.
