Job Description

An exciting opportunity has arisen for a clinical practice lead, based at Chadwell Centre, Paignton working 30 hours per week in the Older Adults community mental health team.

You will be part of the core leadership team.

You will be responsible for overseeing and supporting staff with providing high quality community care for older adults.

You will use your skills and experience to deliver person centred, personalised care which inspires hope and recovery.

You will play a key role in multidisciplinary team working and multiple agencies on a daily basis.

You will lead team members in clinical practice through training, supervision, coaching and mentoring. You will lead the team with embedding clinically effective evidenced based practice.

Main duties of the job

You will carry a small caseload of clients.

To lead on referral triage and allocation decisions.

To be available when required to offer support and advice to clinical staff and able to offer support to duty and in urgent /complex situations.

To support the team with completing complex assessments.

To offer support to clinical staff by offering joint visits to assist in risk formulations and care planning for complex cases.

You will support with identifying team training needs, performance and organise and implement delivery programmes including physical health monitoring and Personalised care framework.

You will be expected to complete audits and identify Quality Improvement factors.

Job responsibilities

The Community Practice Leader is responsible for ensuring compliance with quality andsafety standards within their assigned teams. They will ensure that all individuals receiving care and treatment from OPMH services do so in line with Trust strategic values.

The Key Responsibilities Of This Role Are As Follows

Empower people who use the service to make choices and decisions around their care and treatment

Champion dignity, equality, diversity, choice and respect

Prioritise demand and allocate/delegate accordingly

Lone working at times

Quality improvement and ensuring high standards of clinical effectiveness

Audits

Compliance with CQC standards

Managing complaints and compliments

Learning from experience and support a safe service by ensuring incidents are reported

Supporting learners to have a meaningful placement experience

Advanced clinical delivery and leading others in their clinical practice through training, supervision, coaching and mentoring

Managing a clinical complex caseload

Having a sound knowledge and experience in the Dementia Strategy and embedding this within a community framework.

Making clinical triage decision on receipt of internal and external referrals

Being part of the core leadership team within your allocated team.

Chairing meetings, including clinical and safeguarding

Clinical knowledge and experience of comorbidities and frailty which affect care and treatment associated when treating a mental health condition.

Essential


* 5 years post qualifying work, including community and inpatient mental health
* Risk assessment and management
* Clinical leadership
* Other relevant post registration experience
* Understanding of research principles
* Teaching, training and or supervision of clinical staff
* Experience of working in a senior or advanced clinical role

Desirable

* Ability to interpret research and apply to practice
* Transformational work

Qualifications

Essential

* A professional mental health qualification to degree level or equivalent
* Post Graduate qualification relevant to specialism CBT, NMP, DBT, etc.
* Evidence of commitment to life-long learning and continuing professional development
* Leadership or quality improvement qualification

Desirable

* Management qualification
* Training to level 3 in Safeguarding children and young people: roles and competencies for Health Care staff
