Job Overview
An exciting opportunity has arisen for a Social Worker / Case Assessor to join the Adult Social Care Community Team in Blackpool. This 14-week locum contract offers an hourly rate of £21.07 per hour with part-time or full-time options and a hybrid working model (minimum three days per week in the office).
This is a strengths-based role focused on promoting independence, preventing escalation of care needs, and connecting adults with the right community-based support. You will work closely with service users, families, community organisations, reablement services, and commissioned providers to ensure the best possible outcomes.

What You Will Do
Conduct comprehensive strengths-based assessments of adults to identify needs, risks, and opportunities to promote independence.
Work collaboratively with community and voluntary groups, reablement services, and commissioned providers to create effective support plans.
Use a preventative and solution-focused approach to reduce reliance on long-term care services.
Communicate effectively with service users, families, carers, and professionals to ensure coordinated and person-centred support.
Support individuals to access community resources and informal support networks.
Maintain accurate, timely records and documentation in line with local authority requirements.
Contribute to service improvement and efficient flow through Adult Social Care pathways.
Requirements
Experience working in Adult Social Care, ideally in assessment or front-door services.
Strong understanding of strengths-based practice, prevention, and community-focused support.
Excellent communication, decision-making, and problem-solving skills.
Ability to work both independently and as part of a multidisciplinary team.
