Anexciting opportunity has arisen at Millcroft Medical Centre for an AdvancedClinical Practitioner to join our team.
We are looking for an enthusiastic, motivated and forward-thinkingindividual who can work well as an autonomous practitioner or part of amultidisciplinary team.
Main duties of the job
To workas an autonomous practitioner, working as part of the practicemultidisciplinary team, delivering care within their scope to the practicepopulation. The post holder will be responsible for clinical areas such as, consultingpatients with acute medical problems, health promotion, chronic diseasemanagement, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy andprocedure.

Our current team has a full complement of allied health care professionals including, 7 GP partners, salaried GP's, Advanced Clinical Practitioners, Nurse Practitioners, Practice Nurses, Nurse Associate, Health Care Assistants, Paramedics, SPLW's, First Contact Practitioners, Pharmacists, Business Manager, Practice Manager and a complement of highly skilled support staff.
Job responsibilities
- a.Thedelivery of highly effective medical care to the entitled population
- b.Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice to patients.
- c.Implementand evaluate individual specialised treatment plans for chronic disease patients
- d.Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects on the patients health.
- e.Provideadvanced, specialist nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF
- f.Requestpathology services as necessary
- g.Processand interpret pathology and other test results as required
- h.Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary / specialist care as required
- i.Maintainaccurate clinical records in conjunction with extant legislation
- j.Ensure read codes are used effectively
- k.Maintain chronic disease registers
- l.Prioritise health issues and intervene appropriately.
- m.Support the team in dealing with clinical emergencies
- n.Recognise,assess and refer patients presenting with mental health needs.
- o.Extendedand supplementary prescriber, adhering to extant guidance
- p.Support patients in the use of their prescribed medicines or over the counter medicines(within own scope of practice), reviewing annually as required
- q.Contribute to practice targets (QOF etc.), complying with local and regional guidance
- r.Accept delegated responsibility for a specific area (or areas) of the QOF
- s.Attend and contribute effectively to practice meetings as required
- t.Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
- u.Support the clinical team with all safeguarding matters, in accordance with local and national policies
- v.Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
- w.Prepare and complete the revalidation process
- x.Commit to self-learning and instil an ethos of continuing professional development across the practice team
- Deliver opportunistic health promotion where appropriate
Qualifications
- Registered HealthCare Professional
- Educated to Masters degree as detailed within the HEE document tilted Multi-professional framework for advanced clinical practice in England
- Qualified independent prescriber - V300
- Registered on their appropriate professional register
- Chronic Disease Diploma(s)
- Teaching Qualification
- ALS, PALS
Experience
- Experience of working in a primary care environment
- Experience of working as a practice nurse or community nurse
- Experience of chronic disease management
- Experience of prescribing and undertaking medication reviews
