What you will do:
* Conduct Care Act 2014 assessments for adults with cognitive and/or physical disabilities.
* Develop and review person-centred, strengths-based care and support plans to maximise independence.
* Apply the Mental Capacity Act 2005, completing capacity assessments and best interest decisions.
* Work collaboratively with advocates, families, carers, and multidisciplinary professionals.
* Identify and respond to safeguarding concerns, completing risk assessments and protection plans.
* Liaise with health professionals, occupational therapists, and voluntary sector partners.
* Support transitions from children’s to adult services, ensuring continuity of care.
* Maintain accurate, timely, and high-quality case records in line with Social Work England standards and local policies.
* Engage in supervision, training, and continuous professional development.
