Job Overview
You will work as a Band 6 Senior Community Nurse with the NE Home First Team, providing high-quality, evidence-based, holistic assessments to patients in line with your NMC professional code of conduct and Trust policies.
You will assess, plan, implement, and evaluate packages of care based on individual service user needs, under the guidance of the Team Leader as per NICE guidance.
The team works closely with the Urgent Care Pathway teams – Hospital Liaison and Crisis Response – to respond to urgent and emergency mental health crises.
Assessments can be completed within the community, and the ability to drive is essential. The team has access to a lease car.
You will be responsible for the day-to-day coordination of shifts within the NE Home First Team as required by your manager.
You will collaborate with key partner organizations, service users, carers, clinicians, and other practitioners to deliver a 24/7 service, working shifts where appropriate and operationally necessary.
Shifts are 11.5 hours with an hour break.
Main duties of the job
* Produce accurate and timely documentation for all service users, adhering to professional and Trust policies.
* Triaging referrals and ensuring a timely response and appropriate allocation based on risk.
* Participate in planning and reviewing services for patients awaiting assessment or ongoing care.
* Provide practical support, advice, signposting, and education to service users, carers, and other disciplines.
* Be aware of other statutory and voluntary services for appropriate signposting.
* Record all patient activity on Paris.
* Educate and support service users' understanding of mental illness.
* Support the induction and ongoing development of newly qualified staff.
