We’re offering an exciting opportunity for an experienced and motivated professional to join our Connected Families Service as a Team Leader, supporting adolescents aged 11–17 identified as being on the edge of care. This is a unique role working within a dynamic, multidisciplinary team that’s making a real difference across East Sussex.
You’ll be at the heart of a preventative service that has successfully helped young people remain within their family networks, supporting East Sussex County Council’s wider strategy to reduce the number of children entering the care system.

About the role
You will lead and manage a group of Connected Coaches – non-clinical practitioners providing direct support to young people and families – as well as hold a small caseload of complex young people and/or their parents/carers. The team works intensively with young people experiencing significant challenges, including risk of exploitation, contextual safeguarding concerns, neurodiversity, and mental health needs.
Key responsibilities:
* Provide line management, supervision, and clinical oversight for Connected Coaches
* Hold a small, complex caseload, providing assessment, formulation, and intervention
* Collaborate with Children’s Services to support referrals and urgent cases
* Provide leadership in risk management and safeguarding practice
* Contribute to the development of psychologically informed practice within the team
* Offer clinical advice, consultation and support to social workers across the locality
* Support the Clinical Lead Nurse Specialist and Operations Manager with service governance
About the service
This role is part of the Connected Coach Team, a key component of SWIFT Specialist Family Services – a well-established partnership between Sussex Partnership NHS Foundation Trust and East Sussex County Council.
SWIFT provides multidisciplinary assessment and intervention for families where parental difficulties (e.g., substance misuse, mental health, domestic abuse) are affecting parenting. The team includes social workers, psychologists, psychiatrists, family workers, and mental health nurses, offering a collaborative, specialist response.
T
he Connected Families Clinical MDT also links with SWIFT CAT (Child Assessment and Treatment Team), contributing specialist CAMHS input to complex family systems.
About you
You’re a dynamic, compassionate leader with a background in mental health, CAMHS or safeguarding. You have experience working with adolescents facing complex challenges and understand the importance of multidisciplinary, trauma-informed approaches.
You’ll need:
* A professional registration (e.g., NMC, HCPC or equivalent)
* At least 5 years’ post-qualification experience, ideally in CAMHS or safeguarding
* Experience of working with complex adolescent mental health and contextual safeguarding
* Proven ability to lead, supervise and support a multidisciplinary team
* Strong assessment and intervention skills across a range of presentations
* Ability to work flexibly, including evenings/weekends as part of a rota
