We deliver community and hospital-based care to improve people\'s physical and mental health, and provide specialist support to people with dementia or a learning disability. We work in people\'s homes, in community clinics and bases, with some staff at our 13 community hospitals. Responsibilities
* Assessment, planning and implementation of care, and interpretation of information relevant to the patient\'s condition and progress.
* Identify problems and needs, and set realistic rehabilitation goals.
* Provide specialist evidence-based treatment plans for adults within the catchment area, in their own homes or care homes.
* Work alongside wider multidisciplinary teams within the community, including partner agencies, liaising with the community and acute hospitals, Adult Social Care and Voluntary Services.
* Support assessment, care planning and reablement within the Penwith area (Hayle to Marazion and westwards).
* Contribute to a 7-day rota which includes some weekend and bank holiday working; be flexible with working days within operating hours 8am–6pm.
Working arrangements and location
This is a community-based role requiring frequent travel to various locations throughout the county, including areas not easily accessible by public transport. Successful candidates will need to demonstrate how they will manage travel requirements.
Qualifications and experience
* Experience in assessment, care planning and rehabilitation; ability to work with multidisciplinary teams and partner agencies.
* Ability to provide clinical assessment and evidence-based plans for adults in home or care home settings.
* Excellent organisational skills.
* Open to full or part-time hours; willingness to work across community settings.

This position is open to those who wish to develop skills in assessment, care planning, reablement and rehabilitation within a dynamic, research-active, progressive, integrated, multi-agency team.
