Job overview

We are seeking motivated, dynamic, and passionate Band 5 Community Nurses to work within our South Bedfordshire Primary Home Care Team, based in Dunstable, however, there is an expectation that staff will cover across the bases if needed. This is an excellent opportunity to develop your knowledge and skills within the community nursing setting. You will have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams.

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. Newly qualified nurses who are passionate about a career in the community are welcome to apply.

Detailed job description and main responsibilities

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-21:00.

Education/ Qualification/ Training

Essential criteria

1. Registered General Nurse
2. Demonstrate evidence-based practice and up-to-date clinical skills

Desirable criteria

3. Evidence of post-basic professional development
4. Post-registration qualification in a relevant clinical field
5. Relevant teaching/mentorship qualification or commitment to undertake
6. Post-registration nursing experience, including community experience

Experience

Essential criteria

7. Evidence of and commitment to multi-professional working
8. Understanding of the interface between primary and secondary care
9. Comprehensive understanding of the roles of others in health and social care

Knowledge and Skills

Essential criteria

10. Literate in IT/Computer Skills
11. Evidence of knowledge of clinical governance
12. Knowledge of the principles of Safeguarding of Vulnerable Adults and Children
13. Knowledge of the principles of health promotion and prevention of illness
14. Good oral communication skills based on fluency on the English language

Desirable criteria

15. Demonstrate an understanding of national government initiatives in health care

Freedom to act

Essential criteria

16. Work within clearly defined clinical and professional policies
17. Ability to work independently seeking appropriate support and advice as necessary.
18. Works within codes of practice and professional guidelines
19. Ability to mobilise and stands for most of shift; and kneels and crouches to deliver clinical care e.g. dress wounds.
20. Ability to concentrate on providing clinical care such as drug calculations, carrying out tests and managing interruptions and unpredictable patient/client /carer behavior
21. Occasional distressing or emotional circumstances such as imparting unwelcome news, care of terminally ill or safeguarding issues
22. Post holder could work in occasional unpleasant conditions during home visits and could come across unpleasant body odours, dust, noise/ Body fluid or even verbal aggressions

Other

Essential criteria

23. Current UK driving license and access to a car
24. Ability to travel across sites and across Bedfordshire to meet patient needs and attend meetings, etc.
