Job responsibilities To work within the multidisciplinary Children's Community Specialist Nursing Service under the supervision of the senior case holders in order to provide individualised care to children and young people with complex and acute health needs, as specified by the childs protocols and in line with trust policies. To provide care in the community to children and young people with complex and acute health needs for which training and competency assessment will be required. This may include All procedures relating to naso-gastric feeding and care All procedures relating to gastrostomy/jejunostomy feeding and care Venepuncture/ Fingerprick bloods Wounds and dressings To deliver care to patients, carers and their families adhering to the philosophy of family centered care. To use a range of verbal and non-verbal communication tools to communicate effectively with patients and colleagues to progress their care. This may include patients who have cognitive impairment or difficulty communicating. To act as a chaperone for vulnerable children and young people as per Chaperone policy. Promote the health, comfort, dignity, privacy, and quality of life for children and young people. To empathise with the patient and carer. To be sensitive to individual patient needs, expectations, and cultural beliefs. To work in conjunction with the Paediatrician to promote and facilitate transitions to adult services. To provide distraction and assist in helping children and young people with their coping strategies undergoing any procedures that they may find distressing. Keeping the consultation and assessment rooms and other areas used by the children, young people and their families, or used by the staff on behalf of the patient, clean and orderly. Ensure first line resuscitation equipment maintained in line with trust policy. Assist in maintaining a clean and safe environment. To carry out administrative duties as required e.g. checking stocks, ordering. photocopying / faxing / documentation. To ensure that all duties are carried out to the highest possible standard.
