Role Overview
Band 4 Assistant Practitioner within the Hospital at Home team, part of NHS Grampian. The service delivers consultant‑led, hospital‑level multidisciplinary care to patients in their own homes across Aberdeen City and Aberdeenshire, either as an alternative to hospital admission or to support early discharge from acute services.
Responsibilities
* Contribute to the assessment and treatment of a variety of patient groups, including frailty, acute medicine and respiratory.
* Manage a varied caseload and develop both acute and community working skills.
* Support service developments and work collaboratively with occupational therapists, physiotherapists and the wider multidisciplinary team.
* Work closely with community occupational therapy and physiotherapy teams within Aberdeen City Health and Social Care Partnership and may be required to work within different areas of community physiotherapy including the Community Adult Assessment and Rehabilitation Service and Discharge to Assess.
* Work as part of a weekend rota system.
Qualifications and Requirements
* Experience working within healthcare.
* Meet the requirements of the person specification for the role.
* Valid UK driving licence and access to a car for work purposes.
* Will be responsible for all costs associated with entering Aberdeen City Low Emission Zone.
* Work within the working hours of a full‑time Band 4 role (37 hours per week, to be reduced to 36 hours from 1 April 2026, with an adjusted hourly rate).
Employment Details
Working hours include weekend working as part of a rota system. The closing date is 2 weeks from the date of advert, though the role may close early to applications.
