What You Will Do Conduct comprehensive strengths-based assessments of adults to identify needs, risks, and opportunities to promote independence. Work collaboratively with community and voluntary groups, reablement services, and commissioned providers to create effective support plans. Use a preventative and solution-focused approach to reduce reliance on long-term care services. Communicate effectively with service users, families, carers, and professionals to ensure coordinated and person-centred support. Support individuals to access community resources and informal support networks. Maintain accurate, timely records and documentation in line with local authority requirements. Contribute to service improvement and efficient flow through Adult Social Care pathways. Requirements Essential Experience working in Adult Social Care, ideally in assessment or front-door services. Strong understanding of strengths-based practice, prevention, and community-focused support. Excellent communication, decision-making, and problem-solving skills.
