Responsibilities
* Work autonomously as part of the Integrated Early Years Service (Health Visiting & Family Support), managing a designated caseload of children and families to improve health and support development.
* Collaborate with other agencies to provide high-quality care; participate in safeguarding activities, including gathering and analysing information, making informed assessments, referrals, and contributing at case conferences and core group meetings.
* Support public health initiatives aligned with local and national priorities to promote, protect and improve population health and reduce health inequalities.
* Maintain accurate, comprehensive records and ensure compliance with mandatory training requirements.
* Engage in continuous quality and safety improvement with the team, reflecting on practice and contributing to a culture of improvement.
* Work with a supportive team and benefit from NHS discounts, pensions and development opportunities.

Skills and Knowledge
* Able to communicate with the multi-disciplinary team, present cases, record actions, update the team, and use handover effectively (face-to-face, telephone, and written reports).
* Maintain accurate, timely, concise, and legible clinical records (electronic and handwritten) in line with CNWL policies.
* Work well with team members, showing kindness, reliability and enthusiasm; contribute to team management and mentor/ support others.
Qualifications
* Registered Nurse (NMC Registered)
* Registered Specialist Practitioner Public Health Nurse (Health Visitor)
* Evidence of relevant Continuous Professional Development
Desirable criteria
* Mentorship Preparation Module/ ENB 998/ Practice Assessor
* Nurse Prescribing Qualification
Previous Experience
* Experience of working with complex families
* Experience of multi-disciplinary integrated team working
* Ability to work flexibly and respond to changing demands, both individually and within a team
* Knowledge of Safeguarding Children issues
