About The Role
Are you passionate about supporting people leaving prison to access the healthcare and support they need? Do you have personal lived experience of mental or physical ill health, substance misuse or the criminal justice system? If so, we want to hear from you.
In line with Forward Trusts Equality, Diversity and Inclusion Framework, the RECONNECT service actively welcomes applicants with lived experience to help shape and improve our support for those most affected by health inequalities.
As a RECONNECT Peer Coordinator, you'll lead and support our Peer Support Workers, Peer Mentors and Volunteers across the Eastern region. Youll help bridge the gap between prison and community-based healthcare services, ensuring continuity of care, connection, and confidence in the transition.
Key Responsibilities:
Line manage and supervise Peer Mentors, Peer Support Workers and Peer Champions (Volunteers)
Recruit and train peers, including OCN Level 2 in Mentoring
Facilitate workshops and peer-led sessions, including Meet & Greets for prison leavers
Collect and report on data using Nebula and SystmOne
Promote inclusion by recruiting volunteers from underrepresented groups (Gypsy Roma, migrants, veterans)
Build progression pathways into Forward for people with lived experience
Contribute to the Career Matters: Lived Experience Award evidence base
Maintain strong partnerships with external stakeholders
Ensure safe, holistic service delivery in line with Forwards values and targets
Requirements:
Personal lived experience of physical/mental health challenges, substance misuse or the criminal justice system
Passion for empowering others through mentoring and peer support
Strong organisational skills and experience working with diverse communities
Ability to manage people and projects with a focus on inclusion and impact
Confidence using systems like Nebula and SystmOne (or willingness to learn)
Commitment to equality, diversity and reducing health inequalities
