Pay rate: £27ph LTD

Role and Responsibilities:
1. Keyworker to manage caseload for Older Adults with varying mental health issues, co-morbidities, and complex presentations
2. Carrying out assessments, reviews, interventions and completing discharges alongside relevant documentations to support.
3. Proficient in use of RiO documentations.
4. Administering Depot injections.
5. Completing multi-disciplinary reviews in the community and inpatients on Mental Health Act sections.
6. Liaising with relevant members of the multi-disciplinary team and external agencies to support patient care.
Nursing Qualifications and Requirements:
7. Hold a PIN number as a registered mental health nurse
8. Fully enhanced valid DBS
