About the Role
The Short-term assessment and reablement team in High Peak is seeking a qualified and experienced social worker to join our friendly and supportive team. The role involves managing new adult care cases, conducting assessments to determine care and support needs for adults aged 18 and above.
Essential skills include a solid understanding and experience of safeguarding, capacity assessments in line with the Mental Capacity Act 2005, and the ability to handle a busy workload. Responsibilities include engaging in short-term home care support, general social work duties, safeguarding inquiries, and mental capacity assessments.
Coverage Area
The High Peak area includes Glossop, the center of High Peak, and the southern parts including Buxton.
Candidate Profile
The successful candidate will demonstrate the necessary skills, knowledge, and experience to provide a safe, accountable, cost-effective, and efficient service, helping clients achieve their outcomes. Key responsibilities include carrying out Care Act 2014 assessments and reviews for individuals with complex needs, and arranging appropriate services to meet eligible needs. A person-centered and strengths-based approach with clients and their families is essential to promote independence.
