Position Overview
FULL TIME EXPERIENCED SOCIAL WORKERS needed for Hospital Discharge Team & Community Home First Hub Service.
This is an exciting opportunity for social care professionals to support patients in the acute setting and those in Discharge to Assess & Long-Term facilities. The role involves working across two facilities—one acute and one community—with multidisciplinary teams including therapy, nursing, and reablement services. The focus is on holistic, personalized assessments to ensure safe and timely patient discharges.
Working Conditions
The social care teams work 37 hours weekly, covering 7 days via a rota including days, evenings, and weekends.
Responsibilities
1. Be person-centred, respecting patient choice and control.
2. Promote patient well-being throughout their journey in both acute and community settings.
3. Conduct Strength-Based Assessments and support independence.
4. Complete Care Act 2014 compliant assessments.
5. Source formal and informal provisions supporting patient needs.
6. Collaborate with various professionals daily.
7. Support and guide colleagues, including those less experienced in social care.
8. Maintain knowledge of relevant legislation, policies, and frameworks.
9. Experience with Safeguarding processes and procedures.
10. Perform mental capacity assessments and best interest decisions for patients unable to decide.
11. Understand Deprivation of Liberty Safeguards.
12. Be an active, integral team member.

Additional Information
