As a key "front line" service delivery role within the Brokerage Service, you will be arranging specialist care for children, young people and adult residents with care and support needs and carers in their own homes, in supported living, care homes (residential or nursing) and other place based services including people who fund their own care (self funders).
This essential role focuses on sourcing and securing high-quality placements for vulnerable adults and children, ensuring every service aligns with the specific outcomes identified in individual support plans.
About You
The post holders will have the ability to work under pressure, prioritise tasks, and meet tight deadlines in a fast-paced environment.
Key Skills and Experience Required:
1. Detailed knowledge of Adult Social Care services, such as residential and nursing and Children with disabilities as well as domiciliary and reablement care.
2. Deliver service activities and manage caseloads under the supervision of more experienced staff to ensure intended outcomes for children, young people and adult residents and the Council are delivered within agreed service standards.
3. Sourcing Care Packages: Identifying appropriate services, including residential/nursing homes, domiciliary care, supported living, respite care and other place based services.
4. Negotiation & Value for Money: Negotiating fees with providers to ensure services fit within the council’s budgetary frameworks and providing the best value.
5. System Maintenance (e.g., Mosaic): Accurately recording, monitoring, and updating care packages and purchasing records on IT platforms.

The Operational Commissioning service area within the Commissioning, Insight & Delivery Division is responsible for Brokering services for children and adults who have care and support requirements, processing invoices from a number of providers of social care, providing support for those who want to self commission their own services via a Direct Payment, Shared Lives and LD floating support
