Overview
This post is based at WTWA Wythenshawe. Registered Nurse Band 7. 30 hours Permanent Position.
An exciting opportunity has arisen for an experienced Clinical Nurse Diabetic Specialist to work with cardiac surgery patients, providing expertise in diabetes care by supplying information, support, advice and specialist knowledge to patients, their families and carers. You will act as a resource to the multi‑professional team and participate in coordinating patient care to ensure continuity throughout the care pathway.
Responsibilities
* Coordinate patient care to ensure continuity across the patient’s care pathway.
* Contribute to the review and development of the service to reflect the needs of the patient group.
* Review HbA1c in all new patients on the cardiac surgery waiting list and the non‑elective list; optimise management of existing patients.
* Liaise closely with inpatient wards, outpatient departments and primary care teams, including Consultant Physicians and GPs as necessary.
* Demonstrate knowledge and experience of diabetes management and clinical leadership skills.
* Monitor and improve standards of care through clinical audit.
* Be willing to undertake further training and development to meet the requirements of the post.
* Support the delivery of a high quality, safe and compassionate healthcare service and be a role model, upholding MFT values.
Requirements / Qualifications
* Registered Nurse, Band 7
* Experience as a Clinical Nurse Diabetic Specialist, with relevant expertise for cardiac surgery patients
* Ability to collaborate across clinical teams and contribute to service improvement
