Overview
This is an exciting opportunity to work alongside CAMHS and Adult mental health services to support the transition of young people from CAMHS to Adult services. The post holder will improve the relationship between the two partners and the wider organisation and external partners to best support young people in Croydon. The post holder will be supported by the Service Manager within the Central Locality community team, with supervision provided regularly via the CAMHS/AMHS forum and other reflective platforms. Caseloads are allocated to the CAMHS transition worker following referral from CAMHS, with service users handed over after stabilisation to the Adult community mental health team for ongoing care. Professional and personal development opportunities within SLAM will support career development goals and ambitions for the post holder.
The post holder will be based in the adult service and will directly co-ordinate complex cases and young people from the age of 17 years and 6 months who require enhanced support during transition from CAMHS. They will be responsible for assessing, planning, implementing and evaluating programmes of care for the young people on their caseload. They will ensure the smooth transition of care of young clients from CAMHS to adult services, involving and consulting on transitioning cases between services. They will engage in joint working and consultation on other cases within both services to facilitate sharing and development of skills and knowledge within the teams. They will support the implementation of initiatives that underpin the 16-25 Pathways and Transition from CAMHS to Adult Services, and participate in the continuing development of the interface between adult and CAMHS services.
Responsibilities
* Work in cooperation with the Multidisciplinary Clinical Team and other health and social care professionals to assess, develop, implement and evaluate care programmes to meet the needs of young people.
* Assess newly referred young adults with complex needs into the adult pathway.
* Complete risk assessments following Trust policy and ensure risk assessments are up to date when planning care.
* Maintain accurate records using the Trust's electronic records system, ensuring timely entries for professional access and contract monitoring.
* Outreach hard-to-engage clients, meeting with young people in venues of their choice (home, community, CMHT centre, education settings, work or recreational locations) to aid engagement and participation in treatment and foster independence in daily living.
* Provide clinical supervision and support to junior team members, including supervised visits, skills assessment, appraisal, objective setting and performance management.
* Attend and participate in regular supervision to ensure effective caseload management, appropriate assessment and care planning, and record-keeping.
* Complete carers assessments for all carers of young people on caseload.
* Contribute to service functioning through meetings, taking responsibilities as appropriate, and negotiating with the service manager.
* Contribute to team development through clinical governance activities (e.g., training, audits, statistics, supervision, and ongoing professional development).
* Provide professional, specific interventions within the team while maintaining high professional standards for care and treatment of parents, children, young people and families.
* Act as a transition worker between CAMHS and AMHS to ensure smooth transition for young people aged 17 years 6 months and older who require ongoing interventions from the adult service, with ongoing contact as requested by the young person.
* Build a visible role presence in the community and actively seek contacts with the Voluntary Sector; attend meetings with CVA, BME Forum, Asian Resource Centre and Croydon Neighbourhood Care.
* Conduct assessment work including family liaison / family voice to identify strengths and barriers to engagement in treatment.
* Support the client’s wishes and strengths to be heard, considering difficulties and needs identified during assessments.
* Co-ordinate care for young people post-transition to adult mental health services as required for a minimum of 3 months to ensure continuity of care.
* Undertake other duties as requested by the Clinical Service Lead, Team Leader and CAMHS Team Leader; be prepared to work from any location in the borough.
* Adhere to professional codes of conduct and maintain registration requirements; work within clinical governance policies and guidelines.
* Identify development and training needs with the manager; stay up to date with changing practices and utilise training resources where possible.
* Participate in monitoring, evaluation and audits of the service; promote research-based practice and apply knowledge in daily practice, sharing it with the team.
Qualifications and Requirements
To work in cooperation with the multidisciplinary team to assess, develop, implement and evaluate care; to assess and manage risk; maintain records; engage with clients and carers; and contribute to team governance and development. The post holder should be prepared to work across sites as required and to engage with partners outside the NHS to benefit young people.
