Role Overview
This post offers GP clinical pharmacy services at Cauldwell Medical Centre, with onsite or hybrid options. The successful candidate will support medicines management and lead medicines optimization efforts within the practice.
Responsibilities
1. Work as part of a multidisciplinary team managing chronic diseases.
2. Conduct clinical medication reviews, especially for patients with polypharmacy and complex needs.
3. Support practice staff with prescription and medication queries.
4. Lead medicines optimization initiatives and manage aspects of quality frameworks.
5. Contribute to improving care quality and operational efficiency.
Candidate Requirements
* GPhC registration
* Masters in pharmacy or equivalent
* Experience as a registered pharmacist in primary care or hospital pharmacy
* Independent prescriber status
* Completion of CPPE Primary Care Pharmacy Education Pathway
* Good clinical pharmacy knowledge and communication skills
