Job Summary: As a Complex Care Nurse at Tapera Healthcare Ltd, you will be responsible for delivering high-quality, holistic care to patients with complex medical conditions. You will work closely with a multidisciplinary team to assess, plan, implement, and evaluate individualized care plans, ensuring that the unique needs of each patient are met with compassion, respect, and dignity. Your role will involve providing skilled nursing interventions, managing complex medical equipment, coordinating care with other healthcare professionals, and supporting patients and their families throughout the care journey as well as acknowledging cultural needs.
Key Responsibilities:
1. Conduct comprehensive assessments of patient's physical, psychological, and social needs.
2. Develop and implement individualized care plans based on assessment findings and in collaboration with the patient, family members, and other healthcare professionals.
3. Provide skilled nursing interventions, including medication administration, wound care, tracheostomy care, nasal feeding tubes and other specialized procedures as required.
4. Monitor patients' health status closely, identifying any changes or deterioration and taking appropriate action.
5. Coordinate care with other members of the healthcare team, including physicians, therapists, social workers, support staff, salt team + OT.
6. Educate patients and their families about their condition, treatment options, and self-care techniques.
7. Advocate for patients' rights and preferences, ensuring that their voices are heard and respected in the care process.
8. Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes.
9. Adhere to professional standards, regulatory requirements, and best practices in nursing care.
10.Participate in ongoing professional development activities to enhance clinical knowledge and skills.
Qualifications and Requirements:
* Registered Nurse (RN) licensure in the state/province/country of practice.
* Previous experience in complex care, critical care, or a similar setting preferred.
* Advanced certification in a relevant specialty area (e.g., critical care nursing, rehabilitation nursing) is an asset.
* Strong assessment, critical thinking, and problem-solving skills.
* Excellent communication and interpersonal abilities.
* Ability to work effectively within a multidisciplinary team.
* Compassionate, patient-centered approach to care.
* Commitment to ongoing professional development and lifelong learning.
* Working Conditions:
* Variable shifts, including days, nights, weekends, and holidays, as required.

* UK Driving Licence and have access to a vehicle (Preferred).
