Job Summary
This is an exciting opportunity for an Advanced Clinical Pharmacist (ACP) to develop their career in a primary care setting.
Main duties of the job
* Develop and enhance the clinical pharmacy service to patients.
* Support the practice to achieve maximum performance through QOF.
* Maintain good working relationships with Community Pharmacy.
* Support practices to meet prescribing targets.
Responsibilities include:
* Long-term condition reviews for patients with chronic diseases such as COPD and asthma.
* Conduct clinical medication reviews with patients, providing recommendations on prescribing and monitoring.
* Support patients to optimize medication use, reduce waste, and promote self-care.
* Develop specialist prescribing expertise aligned with clinical priorities.
Provide specialist knowledge on medicines, addressing public health and social care needs to reduce health inequalities.

Person Specification and Qualifications include:
* Pharmacy Honours degree (BPharm or MPharm)
