We are seeking a motivated and enthusiastic individual with a patient‑centred and excellent customer care experience to join the Clinical Administration team as a Care Coordinator & Safeguarding Administrator. You will be based at Farnham Practice and may travel to other sites in Slough to support the clinical administration department.
Main duties of the job
Work with GPs and the MDT team within FRMG practices to assist the ongoing care of special groups of patients (Learning Disabilities, End‑of‑life and Anticipatory Care Planning) and signpost patients to relevant organisations.
Assist the Reception team in navigating patients into different clinical services and in call handling at the Telephony Hub.
Be a central point of contact by managing the safeguarding mailbox and secure emails, processing all information from start to finish with full discretion.
Maintain accurate, secure and up‑to‑date safeguarding records and databases.
Summarise, code and circulate safeguarding documents to the appropriate clinicians, ensuring information is tracked and highlighted where necessary.
Complete administrative reports for external agencies (e.g., MASH, Section 17/47) and prepare documentation for child protection conferences and MDT meetings.
Monitor DNAs for children, refer to appropriate clinicians and engage in regular safeguarding supervision and case reviews.
* Maintain accurate records in EMIS, ensuring patient alerts and warnings are correctly linked across family records.
* Support the practice by contributing to safeguarding audits and data collection.
* Act as a knowledge point for staff regarding local safeguarding resources and updates.
* Assist the prescription team in managing prescription service.
* Assist the docman workflow pathway and admin functions of rota and appointment books.
* Assist in health‑care assistant functions and phlebotomy, including covid/flu vaccinations (after training).
Qualifications
Level 5 qualification (Diploma of higher education or Diploma of further education, Foundation degree, HND or equivalent professional experience).
Experience
* Previous work within a healthcare setting or proven experience in a busy administrative role.
* Confidence in dealing with colleagues from different teams and levels of seniority.
* Experience in community development, adult health and social care, learning support or public health.
* Excellent interpersonal, customer care and communication skills; can work independently or as part of a team and perform under pressure.
* Good literacy and numeracy.
* Initiative‑taking and excellent time‑keeping.
* Good IT knowledge and excellent knowledge of the Microsoft Office suite.
* Ability to build rapport with patients from different ethnic backgrounds and show empathy.
* Experience in a GP practice or working alongside Social Services, Local Authorities, or Childrens and Adult Services.
