An exciting opportunity has arisen for an aspiring individual to develop experience in the specialised area of District Nursing. This opportunity is now available in the Thame/ Princes Risborough area to become a Community Nurse within a newly formed team fixed contract for 12 months. As a community nurse, you would be supported to develop into an autonomous practitioner; nursing patients with multiple complex health care needs in their own home.
Main duties of the job
Community nursing is a specialised area of practice. As a Community Staff Nurse, you will work within a locality team of District Nurse Team Leaders, Community Staff Nurses, Therapists, Assistant Practitioners and Healthcare Assistants, to meet the needs of the complex patient in the community and avoid admissions to acute settings. This includes medication administration, end of life care, managing the deteriorating patient, complex and simple wound care including negative pressure wound therapy and compression therapy, venepuncture, utilising advanced assessment skills and clinical nursing skills. We nurse people with a variety of conditions in a holistic manner in their own home and in residential care settings. Community nurses work in close partnership with social care and other primary health care professionals such as GP’s, Community Therapies and specialised services such as Tissue Viability, Diabetes and Heart Failure.

SKILLS, ABILITIES & KNOWLEDGE
Essential criteria
10. To be able to communicate effectively with patients, relatives, carers and other professionals
11. To be able to care for patients without prejudice.
12. Ability to manage own caseload
13. Knowledge of the Data Protection Act
EXPERIENCE
Essential criteria
14. Demonstrate experience post or pre-registration placements in relevant clinical areas: Care of Elderly Community Orthopaedics Neurology Respiratory Palliative Care Long term conditions
Desirable criteria
15. Desirable but not essential knowledge of working with a single assessment, planning and the delivery of care.
16. Experience of electronic clinical systems and technology e.g. RIO
EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
17. Registered Nurse or student nurse awaiting confirmation of NMC registration
Desirable criteria
18. Committed to own professional development to enhance practice
19. Post registration education and extended skills e.g. Intravenous drug administration, compression dressings.
