Overview
Practice Manager - Changing Futures Sussex, East Sussex County Council. This is a part-time (18.5 hours/week), fixed-term 12-month opportunity. The role may be offered on a secondment basis; please check with your line manager before applying. Location: St Mary’s House, Eastbourne, with flexible hybrid working options (home and community).
Responsibilities
* Lead and inspire a dedicated multi-agency team, including peer navigators with lived experience.
* Champion trauma-informed and relational practice.
* Shape and deliver high-quality support for people with multiple compound needs (MCN).
* Collaborate across health, housing, justice, mental health and social care sectors.
* Drive service improvement, innovation, and performance across East Sussex.
* Manage resources, support development, and embed reflective supervision.
* Ensure safeguarding procedures for adults at risk are followed and integrated into service delivery.
What you’ll be doing day to day
* Lead a multidisciplinary team to deliver integrated support services.
* Embed trauma-informed and person-centred approaches in practice.
* Work with partner agencies to improve outcomes for people experiencing homelessness, substance misuse, mental ill-health, domestic abuse and contact with the criminal justice system.
Qualifications and experience
* Relevant professional and/or management qualification.
* Experience working with multi-agency partners and leading teams.
* Leadership and motivational skills.
* Safeguarding Adults at Risk procedures knowledge.
* Change-oriented approach to work.
* Experience managing a multidisciplinary team is desirable; however, experience collaborating with other agencies to improve outcomes will be considered.
