We have an exciting opportunity for a Support Time & Recovery Navigator to join our Reconnect team in Humberside. Salary - This position is full time and you will receive a salary of £29,263 to £33,510 depending on experience Hours - Monday - Friday: 9am - 5pm Location - Humberside Region How will we support you? What you'll be doing As a Support Time & Recovery Navigator your responsibilities will vary. You will The Reconnect services takes referrals for service users who are leaving the prison establishment within 12 weeks left to serve or up to 28 days post release. The aim of the service is to reduce reoffending by addressing the health and social needs of the service user and ensuring onward referrals are made to ensure continuity of care provision. The key principles of the role are to liaise with a wide range of services such as the police, probation, the court and legal services as well as both inpatient and community health and social care services. you will provide practical support to individuals, maintaining a programme of support, working with their families, advocating and enabling access to agencies including probation services, community-based organisations, clinics, courts, mental health services and hospitals. Experience of working with service users, face to face, with a variety of health needs. Understanding of the complexities of multi-disciplinary and multi-agency working A non-judgmental and compassionate approach Our person-centred model is focused on providing meaningful pathways to anyone faced with criminal charges, as well as understanding what triggers and socio-economic circumstances might lead to people offending. We will treat individuals more holistically, focusing on the person and early intervention strategies to reduce risks and improve outcomes.
