At Downend Health Group we are looking for a part-time CareCo-ordinator (24 working hours per week) to provide support to our patient groups, specifically those withidentified care needs. If you are compassionate, resilient anddedicated to making change, this could be the role for you.
As a Care-Co-ordinator, you will be supported in anestablished team and work closely with GPs and other primary care professionalswithin the PCN. Your role will include ensuring support is madeavailable to our patients and their carers, and that their health needs areaddressed.
You will have excellent communication skills and understandthe need for the strictest confidentiality. We will provide role specifictraining and support for the successful candidate.

Main duties of the job
The successful applicant will support patients in preparingfor or in following-up clinical conversations they have with primary careprofessionals. The applicant will work closely with the GPs and other primary careprofessionals within the PCN to identify and manage a caseload of identifiedpatients, making sure that appropriate support is made available to them andtheir carers and ensuring that their changing needs are addressed.

Job responsibilities
Carecoordinators will:
Proactivelyidentify and work with a cohort of patients to support their personalised carerequirements, using the available decision support aids. For example those patients with a LearningDisability, Mental Health, Dementia and vulnerable/frail patients.
* Bringtogether all of a persons identified care and support needs and explore theiroptions to meet these into a single personalised care and support plan (PCSP)based on what matters to the person, ensuring dignity, choice, respect,independence and rights are upheld at all times.
* Helppeople to manage their needs, answering their queries and supporting them tomake appointments, and ensuring that people have good quality written or verbalinformation to help them make choices about their care.
* Identifybarriers to or issues preventing greater engagement and/or uptake by patientsactively participate in developing solutions for patients and serviceproviders.
* Supportpeople to take up training and employment, and to access appropriate benefitswhere eligible.
* Raiseawareness of shared decision making and decision support tools and assistpeople to be more prepared to have a shared decision-making conversation.
* Ensurethat people have good quality information to help them make choices about theircare, e.g. Support people to understand their level of knowledge, skills andconfidence when engaging with their health and wellbeing.
* Assistpeople to access self-management education courses, peer support orinterventions that support them in their health and wellbeing.
* Providecoordination and navigation for people and their carers across health and careservices, alongside working closely with social prescribing link workers,health and wellbeing coaches and other primary care professionals.
* Supportthe coordination and delivery of Multi-Disciplinary Team (MDT) meetings within practice.
* Supportpatients with poor attendance records in vulnerable patient groups.
* Supportthe paediatric ADHD/Autism (ASD) referral pathway
* Anyother duties as requested by your line manager.
