Social Worker within 18+ Complex Needs Team in Rhondda Cynon Taff UK earning £50.85 per hour on an hourly basis. Ongoing contract. This is an exciting opportunity for a seasoned Social Worker seeking a full-time role to make a real difference in the lives of adults with complex and long-term care needs. We offer the flexibility of working from home with the expectation of attending the office at least once per week. We are seeking someone with experience in adult services and the ability to manage a varied and challenging caseload.

What you will do
* Conduct comprehensive assessments for individuals over 18 with complex needs.
* Develop and implement personalised care plans aiming for positive outcomes.
* Support individuals with both physical and mental health needs in the community.
* Regularly liaise with other professionals and agencies to coordinate interventions.
* Maintain accurate records and prepare reports adhering to high professional standards.
* Attend scheduled meetings at the office and contribute to team discussions and planning.
* Provide direct support and advice to individuals and families to promote their overall well-being.
Requirements of the Social Worker
* Degree or equivalent qualification in social work.
* Current registration with Social Care Wales.
* Substantial frontline experience working in Social Services within the UK.
* Proven ability to handle complex caseloads and make informed decisions.
* Strong values and exemplary interpersonal skills.
