Community Psychiatric Nurse (City West) – Band 6. We have 3 vacancies due to reconfiguration of City Community Mental Health Teams. We are seeking experienced Mental Health Nurses to join City West CMHT.
The CMHT is part of secondary care, providing assessments and treatment packages for service users aged 18–65 years with complex mental health needs. The post holder will maintain a clinical caseload and work effectively in a multidisciplinary team to provide high-quality mental health assessments, treatment programs and risk management. The work involves delivering evidence-based interventions, engaging patients and carers, and promoting recovery-focused care. You will visit people in their homes and communities, as well as run clinics at the team base and other community venues.
Experience of post-registration work is preferred, ideally with this client group. We are a dynamic, supportive team that values clinical supervision, including weekly group psychology supervision. The team discusses complex cases and explores new initiatives in mental health.
Main duties City West CMHT provides holistic, person-centred mental health services for people aged 18–65. The CMHT is a well-established multidisciplinary team, including Consultant Psychiatrists, Nurses, Healthcare Support Workers, Occupational Therapists and Psychologists. The team delivers face-to-face assessments and care for service users referred via Primary Care, In-patients and Crisis Teams. The successful candidate will deliver effective healthcare within the team by performing comprehensive assessments of nursing needs, planning, implementing and evaluating care delivery, and taking responsibility for the care needs of individuals and groups.
The key skills required include ongoing assessment, risk assessment, problem-solving and excellent communication. The post holder will collect, collate and report patient information, maintaining accurate nursing records in line with NMC and Trust standards, and involve clients/service users and carers/relatives in assessment, planning, delivery and evaluation of care.
