Overview

KCHFT are looking to recruit a Pharmacist who is looking for a development opportunity. This role will enable a competent 8a pharmacist to gain the necessary leadership and strategic skills to develop, within a maximum of 18 months, into an 8b pharmacist, who can demonstrate leadership competencies against the NHS Leadership model.

We will agree with the post holder a personalised development programme that addresses their specific development needs to meet a competency assessment within a maximum of 18 months for the 8b role.

While you are developing you will be mentored by the Deputy Chief Pharmacist. In this role you will be supported to lead the pharmacy frailty enablement teams within the community hospitals and domiciliary settings (Integrated Pharmacy Team‑IPT). These are currently 2 teams of pharmacists, technicians and pharmacy support workers, one within each setting.

You will be supported to develop and implement a highly specialised integrated clinical pharmacy frailty service within these settings, ensuring that the team work to the same aims irrespective of the setting, with a patient and MDT centred focus.

This is also essential with external partners in particular the Healthcare Partnerships (HCPs), Integrated neighbourhood teams (INTs) and PCN MDT members. You will need to be flexible in your approach to ensure that the team adapts to the evolving NHS landscape.


Responsibilities

* Lead the pharmacy frailty enablement teams within the community hospitals and domiciliary settings.
* Develop and implement an integrated clinical pharmacy frailty service within these settings, ensuring that the team work to the same aims irrespective of the setting, with a patient and MDT centred focus.
* Develop collaborative relationships with internal KCHFT clinicians and frailty service leads to understand where the team can add most value.
