Rate: £284 per day

Responsibilities:
4. Conducting comprehensive assessments of adults with disabilities, identifying needs, strengths, and risks
5. Developing and implementing personalized, innovative care plans that promote independence, well-being, and transformation
6. Providing ongoing support and intervention to adults with disabilities and their families, ensuring access to necessary services and resources
7. Collaborating with healthcare professionals, community organizations, and other relevant agencies to coordinate comprehensive and transformative care
8. Maintaining accurate and up-to-date records of assessments, interventions, and outcomes in line with legal and professional standards
What we are looking for in a Social Worker:
9. Social Work England registration
10. Eligible to work in the UK
11. Hold a full UK licence
12. Post-qualified experience
13. Degree level or equivalent in Social Work
